If you have not yet registered for this service, please complete the registration form below, then email it along with a picture of your ID to branchmp@lanarkshire.scot.nhs.uk. We will respond with your unique registration token and details on how to log in. Please note that a unique email address must be used per patient who wishes to register i.e. you cannot use one email address for more than one patient.
